Cervical Facet Syndrome

 

Cervical facet irritation or damage can cause cranial, cervical or upper shoulder & back pain referral.

 Potential Causes: 

 

  • Neck Injuries
  • Cervical Disc Injuries
  • Whiplash, sprain/strains
  • Osteoarthritis or Rheumatoid Arthritis
  • Repetitive Stress & Poor Posture due to occupation

 

History:

 

  • Dull, achy pain although it may be sharp during acute episodes
  • Headaches & Limited range of motion
  • Pain is localized and patient can often pinpoint the pain
  • Neck muscle spasm
  • Sometimes radiates to shoulder or mid back region
  • Pain with extension & rotation
  • Antalgic neck posture (neck slightly flexed and laterally flexed)

 

Treatment:

 

  • Acute Phase – Ice to reduce swelling & muscle spasm
  • Subacute Phase – Rest, heat, hydrotherapy, ultrasound (avoid heavy lifting & rapid neck movements)

 

  • Cervical Adjustments, Mobilizations & Distraction
  • Massage Therapy (ART-Active Release Technique, Swedish) & Muscle Stretching
  • Electrotherapy (Ultrasound, IFC, TENS) – Reduces painful muscle spasms
  • PassiveRange of Motion & Active Range of Motion exercises in a pain-free range
  • Strengthening should being with isometric exercises & progress to isotonic
